A common and serious issue where fuel injector seals fail, allowing combustion gases to escape. Called "Black Death" due to the black carbon deposits that form around the injectors.

Solution
Replace failed injector seal(s) and clean all carbon deposits. In severe cases, injector(s) may need replacement. Professional cleaning of the injector bore is recommended.
